Medication delivery is available for clients 55+ that do not have means to pick them up at Trenton Pharmacy. If you have an appointment at the clinic you must wait for your medications. The last medication run of the day is at 3:00 P.M. DO NOT wait until you are out to reorder your medication(s). There is a 24 hour notice required for the delivery of medications. Due to the volume of medication deliveries, CHR’s will attempt to deliver once. If no one is around during the time of the delivery, medications will be returned to the pharmacy and will need to be picked up.
Montana clients: CHR’s go to Sidney every Tuesday, and Plentywood on Thursdays. You must order your medications the week before if you want CHR delivery.
Transportation is also available to clients 55+ and those who are disabled that have NO OTHER MEANS of transportation. Request for transport is to be done 24 hours in advance. If you schedule a transport and no longer need one, please contact the CHR office at (701) 774-0303 to cancel. It is the responsibility of the client to call the office and schedule the ride. Please do not schedule with the driver! If you are requesting transport outside of the Trenton Service Area, we need an immediate notification. Please let us know if the appointment will run into the next day so proper arrangements can be made.
You may also be eligible for the Title VI program if you are 60+. Title VI includes all services listed above, including Wellness Visits, Meals on Wheels, Senior Center Meals, Caregiver Assistance, and Lending Tree accessibility.
Wellness visits with CHR staff can be accommodated based on client needs. We offer a variety of routine scheduled options such as weekly, every other week or monthly based on client preference. During this time CHR’s will arrive at your household and provide a wellness visit, taking vitals and conversating about your overall health. Please call our office if this service interests you so we can add you to our rotation
Meals on Wheels is a meal service program for qualifying home-bound clients 60+, cancellation of meals must be given within 24-hour notice. The Senior meals are now being cooked and served out of the TISA kitchen. That address is 331 4th Ave. East, Building #1. The suggested donation for 60+ is 5.00 and for 59 and younger, tribal and non-tribal is 10.00. Must let the cook know by calling 701-572-8316 and asking for Wanda or by calling our office at 701-774-0303 by 2 pm the day before for the next day’s meal.
Caregiver assistance is also provided to caregivers 60+ taking care of someone 60+ OR disabled. Assistance is provided by Title VI staff to relieve caregivers as needed. The Lending Tree lends medical household items such as wheelchairs, walkers, canes, shower chairs, transfer benches, etc
